Abstract:

Background: Glucagon-like peptide-1 receptor agonists (GLP-1 RAs) have transformed obesity management, achieving sustained weight reduction and metabolic improvement. However, rapid adipose tissue loss may contribute to dermal laxity, reduced skin elasticity, and aesthetic dissatisfaction. Understanding the structural changes in the dermis during metabolic weight loss is essential for developing preventive aesthetic strategies.

Objectives: 1 To evaluate dermal structural changes associated with GLP-1–induced weight loss.

2 To analyze collagen remodeling and elastin response during metabolic treatment.

3 To propose an integrative aesthetic protocol to preserve skin firmness.

Methods:  Observational clinical evaluation of patients undergoing GLP-1 receptor agonist therapy for obesity and overweight. Skin quality parameters were assessed using clinical examination, photographic documentation, and elasticity-based evaluation. Interventions included early collagen biostimulation, skin tightening protocols, and nutritional support.

Results: Patients receiving early aesthetic intervention during GLP-1–induced weight loss demonstrated improved skin firmness, reduced dermal laxity, and better overall aesthetic outcomes. Collagen-supportive protocols appeared to mitigate the negative aesthetic effects of rapid adipose tissue reduction.

Discussion: Rapid weight reduction leads to decreased mechanical tension in the dermis and subcutaneous tissue, affecting fibroblast activity and extracellular matrix integrity. Proactive aestheticinterventions, including collagen induction therapies and biostimulators, may help maintain dermal structure and improve patient satisfaction.

Conclusion: GLP-1 receptor agonists represent a paradigm shift in metabolic medicine. A multidisciplinary approach combining metabolic therapy with proactive aesthetic protocols is essential to preserve skin quality and optimize outcomes in patients undergoing significant weight loss.

Biography:

Dr. Karolina G. Landaeta, MD, MSc, is a physician with more than 21 years of clinical experience in aesthetic and metabolic medicine. She is the founder of the concept “Metabolic Aesthetic Medicine,” an integrative approach that explores the relationship between metabolism, weight loss, skin physiology, and aging.

Dr. Landaeta has treated more than 15,000 patients throughout her career and has participated in international medical conferences in Europe  and Latin America as a Speaker. She completed advanced academic training in aesthetic medicine at the University of the Balearic Islands, Spain, and pursued continuing medical education at Harvard Medical School in diabetes, obesity, and overweight management.

Her current research focuses on GLP-1 therapies and the preservation of skin quality after weight loss, bridging metabolic health with regenerative and aesthetic medicine.
